Likely cause
ActivClient Advanced Configuration default.
Fix
1. Start > All Programs > ActivIdentity > Advanced Configuration Manager.
2. Select 'Notifications Management'.
3. Double-click 'Display No Smart Card Reader Alert' so it flips from YES to NO.
