Symptoms

A previously-working CAC site is suddenly unreachable on the Mac with no obvious change.

Likely cause

Stale Identity Preference entry in the login keychain is binding the site to a cert that is no longer valid.

Fix

1. Spotlight > Keychain Access. 2. Select 'login' under Keychains and 'All Items' under Category. 3. Click the Kind column to sort and scroll to 'Identity preference' entries. 4. Select the entries for the failing site and delete them. 5. Eject the CAC. 6. Safari > Clear History > all history. 7. Safari > Settings > Privacy > Manage Website Data > search the host (e.g. mail.mil) > Remove. 8. Reinsert CAC and retry. On Big Sur / Catalina / Mojave check the 'Certificates' category instead of 'Identity preference'.
