Symptoms

AKO or AMID throws 'Insert card' although ActivClient sees the CAC fine.

Likely cause

IE's personal cert store is corrupt or the cert is not bound to Windows.

Fix

1. IE > Tools > Internet Options > Content > Certificates > Personal > delete all listed certificates. 2. Eject and reinsert the CAC - IE should auto-reload the certs. 3. If not, open ActivClient > Tools > Advanced > Make Certificates Available to Windows. 4. Retry.
